Application forms are available in Block – A or anyone can download the same from website. (i.e www.matriz.in)

For on-line admission log on to www.matriz.in

Applications duly filled in prescribed form should be submitted at Block – A .

DOCUMENTS REQUIRED

1	Three attested copies of 10th or equivalent admit card as proof of age.
2	Three attested copies of marksheet of 10th or 12th or equivalent.
3	Caste Certificate for claiming belonging to ST/SC/OBC/MOBC from appropriate authority as decided from time to time.
4	Six copies of recent stamp color size photographs ( not attested ).
5	Original documents for proof of Educational Qualification and age should also be submitted which will be returned after verification ( only for students admitted into civil branch )
